Form 1095-B Information

 

IRS Form 1095-B is used to report certain information to the IRS and to taxpayers about individuals who are covered by

minimum essential coverage. This form communicates to the IRS if and when you had coverage through Direct Fulfillment’s health plan during 2019.

 

Because the “individual mandate” portion of the ACA was repealed for 2019, Form 1095-B is no longer required to compute and complete your federal tax returns.

 

The IRS has declared that companies such as Direct Fulfillment are no longer required to provide Form 1095-B to eligible employees. Direct Fulfillment has decided to still provide Form 1095-B to eligible employees. If you were enrolled in the Direct Fulfilment health plan at any point in 2019, you should be receiving a 2019 Form 1095-B next week.

If you request a new or amended Form 1095-B, Direct Fulfillment will provide you with the form within 30 days. Please note, the Form 1095-B is not required to complete your federal or state tax returns! The Form 1095-B is an information only form.
